Traditional approaches to islet transplantation have suffered from either a complete lack of vascularization (i.e. encapsulation) or delayed and often insufficient vascularization (e.g. Edmonton protocol), both of which result in a loss of functional islet mass. This project aims to create a tissue-engineered construct that enables immediate perfusion by vascular anastomosis coupled with immune protection by isolation from lymphatic surveillance. Pilot studies support the plausibility of this project, but substantial challenges remain.
